The correct answer is both. When they occasionally do get things right it's because they tried to mess up out of malice, but they messed up the malicious mess up out of incompetence. The only other time they get things right is when they do nothing, which is unfortunately even more rare.

Yes, you tend to get some incompetence at the local level usually. Once you get to the standards bodies (BIS, FSB, WTO, WHO, OPEC, etc.), they tend to have access to practically all of the data they need to run complex simulations and probably malice >> incompetence.
