i have a really big beef with this. i have a genetic mutation that causes what you call "narcolepsy" - i had a notable "ragdoll reflex" as a child that when you picked me up, i went slack like a ragdoll. i don't have normal sleep patterns, i can't live the nine to five because i often need a nap in the middle of the day. the stress of dealing with this can be resolved by a medicine - GHB. but then they only allow you to have the one with sodium. sodium oxybate. but if i also concurrently have issues with my kidneys. then what? what if my blood pressure and CNS arousal are in hypervigilance? all htat sodium is going to give me a heart attack. there is also calcium oxybate, but regulations put that out of reach for me.
i used to have more often a problem with sleeping a lot. and now i have got older and lived through too much stress, i have the opposite problem - every day i'm barely getting 5 hours a day. and i smoke cigarettes, drink coffee and alcohol to medicate myself for that, which is teh only options i have that don't put me in the crosshairs of medical monopoly enforcement (aka prohibition).
fortunately for me, it seems that this is actually about diet - i can't tolerate carbs, it kills my kidneys, denies energy to my brain because my BBB doesn't have normal ingress for sugars. my body needs to be running on ketones. alcohol can also bypass the same way and jump through the barrier and become a fuel source for my carb-denied brain.
but none of that is common. maybe not n=1 but there is other signs. i don't get sympathetic effects from morphine, for example. it suppresses my breathing, but it doesn't cause flushing and sedation. all of the medical models work on probabilities and everyone at the edges is treated like they are incurable.
